Continuous learning should be focused on prevention and proactive measures to identify defects before they cause harm. Quality and safety are the proactive arms of Risk Management. Managing adverse events is reactive. Ideally, your program should be ninety percent proactive and ten percent reactive. This is why safety culture and reliability are essential. Learning organizations concentrate their attention on searching for better ways to improve the lives of patients, families, staff, and the community.
We generate a mountain of data and information daily at every level of the healthcare system. However, many of these systems are not integrated and the data generated is not converted into a useful format for clinicians and staff to act upon in a timely manner. Business Intelligence experts are needed to collect the data in a standard way, with common definitions, and produce dashboards and projections to assist in making improvement decisions that most benefit patients and the organization.
Continuous learning requires feedback loops that provide data into the reporting systems to inform and generate insights and prompt action and learning.
